HPC Campus Usage Form

To request usage of the HPC Campus, please fill out and submit this form.

We request a 2 week notice please.

(Tables & Chairs are located in the "Table & Chairs Room" in the MPR)




* All ministries are responsible for set up, tear down and clean up of all equipment used and coffee carts each time facility is used. In Multi-PurposeRoom--follow the "Set-up Diagram" located on the wall near the thermostats.

  • Return items back to where you found them & re-set all rooms used
  • Clean tables, counters, dishes, etc. & put away
  • Follow Kitchen & MPR Clean-up guidelines(if using)
  • Vacuum floors- if necessary
  • Check Restrooms for cleanliness
  • Turn off lights, heating/air back to normal setting, lock doors
  • Partitians will be changed by authorized personnel ONLY
  • Must have prior approval to use Media equipment
  • (Cleaning supplies & vacuums are located in closets in MPR)
