Section A - To be completed by 1st year moms

Please make your intentions clear by selecting form the coices below. If you choose to accompany your child to Colorado at the expense of the Knights of Heroes Foundation, you must take part in the moms program.

If you have a younger sibling accompanying you to Colorado, please list them by name and age:

If you elect to take part in the moms program, you are required to attend the group dinners and are encouraged to take part in as many of the following optional programs that you desire. Please indicate which of the optional programs you would like to take part in while in Colorado (you can take part in all of them).

If you are planning to white water raft, please select which trip you would like to take - Bighorn Sheep Canyon: Beginner and intermediate skill level trip. Minimum age is 6, minimum weight is 50 pounds, must be able to swim. If you are taking your child on this trip, you must accompany him/her. - Royal Gorge: intermediate and advanced trip. Minimum age is 14. Note: we will have child care available if your child does not want to go rafting with you.

If you are bringing a younger child to the moms program, please provide details on what you would like your child(ren) to do on rafting day. For example, "I request child care for my younger child or, my younger child will accompany me on the rafting trip"

Section B - To be completed by moms attending for their 2nd year.

You are welcome to accompany your child(ren) to Colorado. KOH will cover your airfare and provide you lodging at the Homewood Suites off base. You will not have any scheduled events. Feel free to do whatever you desire all week. One 2nd year mom will take the lead in coordinating group events, all events are optional and you will be responsible for the cost of these events. Please make your intentions clear by selecting from one of the choices below.

Section C - To be completed by moms attending for their 3rd year or more.

You are welcome to accompany your child(ren) to Colorado. You are responsible for your own airfare and lodging accomodations and cost. For those who raise $1000 for KOH before 1 April 2018, KOH will cover the cost of your airfare and lodging. Please make your intentions clear by selecting from one of the choices below.

Camper conduct policy. The Knights of Heroes has a zero tolerance policy for camper misconduct. Misconduct includes obscene language, derogatory comments towards others, and physical violence. If your camper does not adhere to these standards, he/she will be sent home immediately (unaccompanied campers) or sent back to Colorado Springs to spend the remainder of the week with you. Please make this clear to your camper(s) and ensure they are prepared to maintain this standard of conduct prior to committing to attending the program. By checking the box below, your camper is agreeing to these standards of conduct.

Please remember that we have a very limited number of spots available every year and if you commit and do not attend, you have a left a spot vacant that another family could have filled. Additionally, there is a financial loss to the Knights of Heroes Foundation of roughly $1000 for every child who fails to attend.
