Knights of Heroes Foundation / How To Apply / Pre Camp For Add'l Camper

This form is to be used for moms who have more than one camper attending. If you have only one camper attending, the Pre Camp Accept Form is all we need.

Activities child participates in throughout the year (sports, drama, band, etc..)

Would you describe your child as an introvert or extrovert?

Does your child prefer group or individual activities?

Is your child comfortable talking about their deceased parent ? (Sharing fond memories, expressing their feelings on their loss etc.) Or does your child avoid the topic?

Academically, how is your child doing in school? (Grades, activity involvement, parent-teacher feedback etc.)

Is your child experiencing any social difficulties? (Bullying, alienation from peers, self-isolation etc.)

Please list three things you would like to see your child gain from their KOH mentorship:

Would you like for our mentors to provide Christian mentorship to your child. If so, in what manner (prayer, bible study, etc..)

Does your child have any dietary restrictions (allergies, gluten free, etc.)
