In order to be approved to work in our Children's Ministry, a criminal background check is necessary for anyone 18 years of age and older. The following information is required for this process.

Your Social Security Number is required to run a background check. Once Pastor Brian receives this form, he will call you for that information.

Due to the complex problems we each face in today's world, we need a statement from you in answer to the following question. Insurance companies request that church groups, as well as other groups, follow this procedure to eliminate charges against church-related programs.
